New Patient Forms – Princeton Physical Therapy & Sports Medicine
To help us provide the best care possible during your first visit, all three patient forms must be completed prior to your initial appointment. This includes the:

If you are using an iPhone, iPad, or Android device, please follow these steps to ensure your information is saved correctly:
- Download the App: Download the free Adobe Acrobat Reader app from your App Store or Google Play Store.
- Save the PDF: Click the link below to open the form. Tap the Share/Export icon and select “Save to Files” or “Download.”
- Open in Adobe: Open the Adobe Acrobat app, find the file you just saved, and tap to open it.
- Fill & Sign: Tap the blue “Edit” or “Fill & Sign” icon. You can now type in the boxes and sign the form.
- Submit: Once finished, tap the Share icon in the app to email the completed form back to us at (Your Email Here).
⚠️ Note: Mobile browsers (like Safari or Chrome) often look like they work, but they usually won’t save what you type. Please use the Adobe app to avoid losing your progress!
You may either:
✅ Bring the completed forms with you to your first appointment
OR
✅ Fill them out online, save them as PDFs, and email them ahead of time
If you choose to complete the forms online, please be sure to:
- Download or PDF print each completed form
- Save the files to your device
